Faculty sometimes move on to new opportunities—whether it’s a new job, retirement or a well-deserved break. We want to make your transition as smooth as possible. Follow these steps to prepare.

Two months before leaving

Start by completing these tasks. Afterward, your division will give you a list of tasks with deadlines.

  1. Contact your benefits and payroll specialist
  2. Notify your division chief and administrator of your departure
  3. Submit to HR your resignation/retirement letter with the following information:
    • Name
    • Last day onsite
    • Last day in clinic (if applicable)
    • Last day on payroll
    • Type of departure (retirement or resignation)
  4. Contact your research administrator (if applicable)

On your last day

  1. Set up an out-of-office email message with your departure date and who to contact for questions regarding clinic, administrative work, and research.
  2. Return all department property to your division's administrative team, including but not limited to:
    • Laptop, iPad, or other devices
    • Office keys
    • Lab coat
    • UW Health badge
    • Parking permit(s)
    • Telephone cards
    • Purchasing or corporate credit cards
    • Any other Department-issued items
