SMPH Onboarding
Follow this timeline and complete tasks promptly.
Department Onboarding
- Work with your division administration to collect and set up your laptop and get building access.
- You will be taken to pick up your UW Health ID badge from the badge office at G5/146 at the UW Health University Hospital. The badge office hours are Monday – Friday, 11 am – 3 pm.
- Schedule a standard headshot portrait.
- Submit information with this form to set up a bio page in the DOM directory. (Example)
Division, Clinic and Hospital Onboarding
This applies to UW Health, William S. Middleton Memorial Veterans Hospital and UnityPoint Health–Meriter settings.
Each new faculty member will be provided an itinerary outlining whom they will meet throughout their first week. Within will be a list of other tasks and assignments required for setting a clinic schedule and division assignments.
