This document includes user request forms for the Dept of Medicine IS Group.
Information Systems Forms
The New Employee Authorization Form is used to grant employees who receive helpdesk services access to Dept of Medicine workstations and services.
The Affiliated Access Form is used for account creation for affiliated appointments with Department of Medicine that need access to Department of Medicine resources but not are not employees of the department nor do they receive any helpdesk services. The data custodian or Division Administrator must approve the access prior to the account creation.
The Terminated Employee Form is used to end access to Dept of Medicine workstations and services.
The Off-Campus Equipment Form is used to clear departmentally-purchased equipment for off-campus use.
The Specialized Computer Service Request Form is used to detail a non-standard IT service request. Users can contact the Help Desk for further information.
The VPN Authorization Form is used to grant users access to the Dept of Medicine VPN environment for remote access solutions.
